As part of this re-accreditation process we may ask you to participate in a confidential patient survey.

These surveys are aimed at collecting your views on our practice so we can continually improve our services.

At this site visit another General Practitioner other than our own and a non-GP surveyor visit us.

Your medical case notes may be requested to be viewed as part of our accreditation.

Please Advise Reception if you do not wish your notes to be viewed as part of our accreditation process and we will flag your file. WE RESPECT YOUR PRIVACY
